The short version

This website has one form. If you fill it in, we get what you typed so we can reply. We do not sell it, we do not share it with advertisers, and we do not use it to follow you around the internet.

What we collect

When you contact us. The name, phone number, email address and message you enter into our contact form. You choose what to put in the message field. Please do not include detailed medical information there; a phone call is the right place for that.

When you simply visit. Our hosting provider records standard server information such as IP address, browser type and the pages requested. This is ordinary web-server logging used to keep the site running and secure. We do not combine it with anything you send us.

What we do not do

• We do not sell or rent your information.
• We do not run advertising pixels or retargeting on this site.
• We do not set marketing or tracking cookies.
• We do not have a patient portal here. Nothing clinical is stored on this website.

Health information

Collier Concierge Physical Therapy and Performance PLLC is a healthcare provider. Clinical records created in the course of treating you are protected health information and are governed by HIPAA and Florida law, not by this page. This policy covers only the website.

Email and web forms are not secure channels. Please do not use them to send medical details, images of an injury, or anything you would not want read by someone else. Call us instead.

How long we keep it

Enquiries that do not become care are kept only as long as needed to follow up, then deleted. If you become a patient, your clinical record is retained for the period Florida law requires for physical therapy records.

Your choices

You can ask us what enquiry information we hold about you, ask us to correct it, or ask us to delete it. Call 941-275-9293 or email [email protected] and we will handle it. There is no form to fill in and no fee.

Children

This site is intended for adults arranging care for themselves or a family member. We do not knowingly collect information from children through this website.

Changes

If we change how this site handles information, we will change this page and update the date at the top.
